Once you have scheduled your appointment, you will receive an email with information on setting up your patient portal. Your patient portal will be where you fill out paperwork, send/receive messages to and from the doctor, view labs and growth charts, and schedule subsequent office visits.

Please fill out all patient questionnaires prior to your first visit. Bring any important medical records, labs, vaccine records, and a complete list of medications and supplements to your first visit. We look forward to meeting you!
